WHO WE ARE:

Trinity Health Associates LLC opened in 2019 as a Care Management Practice that currently operates in Texas, Oklahoma, Arizona, Arkansas, New Jersey, Michigan, Illinois and Georgia. We deliver Chronic Care Management, Remote Patient Monitoring, Behavioral Health Management, Principal Care Management, Transitional Care Management, Comprehensive Medication Reconciliations, and Health Risk Assessments to our patients with the focus of preventing hospitalizations and improving overall health and wellbeing. Our Multi-Level Care Management program educates patients on their health and provides tools and resources to safely manage in an independent home environment.

The long term vision of Trinity Health Associates is to provide the best quality of care for our patients while making a difference in their lives and health, create a positive community for our employees and be recognized as leaders in healthcare. We aim to decrease the total number of hospitalizations by empowering each patient towards a greater state of independence and improved health all the while being a place that employees love to work. We also want to ensure that our doctors lives are easier and that our superior Chronic Care Management positively effects their practice.
