Trinity Health Assocaites provides you with a devoted Health Care Professional that works directly with your physician. Our Care Clinician will establish a relationship with you, develop an individualized plan of care then follow up monthly to help you progress through a healthy life.

Managing your health care can be difficult, but it doesn't have to be. Trinity Health Associates provides Chronic Care Management that can help connect the dots so you can spend more time doing what you love.

Provide you at least 20 minutes a month of specialized care management

Provide you with 24 hour telephone access to a knowledgeable health care professional

Coordinate and communicate with your pharmacy, specialist, testing centers, hospitals and more

Help you obtain outside resources you may need while coordinating community and social support services

Contact you via telephone between your doctors visits to help keep you on track
